ANNUAL CONTRACT REVIEW AND REQUIREMENTS

All of the following steps must be completed annually to stay in compliance with your contract and continue working for our agency:

ANNUAL CONTRACT REVIEW TO BE COMPLETED BY DECEMBER 31ST OF EACH YEAR

1. Have the following required documents ready to attach to the DocuSign Annual Contract Review Packet:
      * Valid State / Federal ID or Driver's License
      * Valid Auto Insurance (If you have a vehicle)
      * Valid Vehicle Inspection Sticker (If you have a vehicle)
      * Valid First Aid Credentials (either a First Aid Card or complete the First Aid Credentialing via Petra College)

2. Complete the Annual Contract Review packet emailed to you from our office.

3. Complete the Annual Renewal of the State Mandated DSW credentialing:
